Year to date total hospital acquired pressure ulcers = 22 There were 3 hospital acquired category 2 pressure ulcers in December, all graded as no harm.

Year to date total in-patient falls with harm = 32 There were 2 in-patient falls with harm in December

All falls resulting in harm are reviewed within 7 days using a screening tool. If the screening tool dictates then it will proceed to a full root cause analysis investigation.
