No harm Low Moderate Severe Death Total

An increase from 2018-19 is shown with minor (low harm) and no harm incidents, whereas the number of moderate incidents and deaths has reduced and major incidents remains the same. The increase in low level and no harm is attributable to the following reasons • Increased reporting culture – supports learning from incidents to prevent them happening again • Continued increase in number of reactions to chemotherapy treatment (reportable in line with NRLS guidance). Treatment given as intended, no error has occurred • Change in incident reportable radiotherapy guidelines.
