Refine and improve processes and systems to build effective risk management
This is important to ensure risks are identified, assessed, recorded, mitigated and reviewed at an early stage to prevent unnecessary adverse events DEVELOP A TEAM DEDICATED TO MAINTAINING EFFECTIVE RISK MANAGEMENT ‘THE CHRISTIE RISK CHAMPIONS’ How will we do it; 1. SCOPE and identify the role and responsibilities of The Christie Risk Champions 2. COLLABORATE with all The Christie Risk Champions with the following elements; 2.1 REVIEW the process of adding risks to the risk management database and how risks are managed by divisions and their departments 2.2 REVIEW the risk grading matrix contained within the risk management strategy to ensure it remains current 2.3 ENSURE all risks on risk registers are underpinned by a risk assessment that has been approved at a departmental or divisional meeting, according to grade. No new risks will be added to a risk register without an underpinning risk assessment. 2.4 UNDERTAKE an audit of risk assessments underpinning risks on registers 3. MAINTAIN assurance that all Trust committees pertinent to the management of safety undertake a yearly review of their effectiveness and review their terms of reference and address any issues identified 4. STRENGTHEN the process of monitoring and compliance with the risk management process within Datix including an automated reminder system within Datix when risks require a review 5. INCREASE visibility of the weekly Executive Review Group meetings by providing an open seat to all levels of staff 6. STRENGTHEN the communication systems to ensure that learning is cascaded throughout the Trust and fully embedded in all areas by developing a framework to support this
