1.1 - Incident Reporting

The severity of major and death relate to know complications and clinical events. All are progressing through the investigation process prior confirmation of being directly attributable to the final documented harm.

There has been a slight increase in reported patient safety incidents in March, contributed to by a change in internal processes for monitoring and reporting of hospital acquired infections.
