In summary over the last six months, 50% of concerns have had an element relating to attitudes and behaviours. This compares with 39% and 42% for the previous six months. 21% related to policies, procedures and processes. One staff member reported a patient safety concern, these had already been reported to the manager. Their concern related to their belief that they had not been heard as well as being treated differently as a consequence. This is part of a wider HR process and case is being reviewed by a senior manager.
